Notice of Privacy Practices for COVID-19 Symptom Check

University of California, Santa Cruz COVID-19 Symptom Checking Officer is requesting this information as part of campus efforts to comply with federal, state, and local public health agencies in order to further slow transmission of Novel Coronavirus Disease 2019 ("COVID-19").

The principal purpose for which this information will be used is to establish a safe work environment for UCSC employees, to reduce the risk of COVID-19 spread, to support contact tracing of possible exposure, and to support employees who have been exposed while at work.

Foreseeable future disclosures include:

  • Santa Cruz County Public Health Agency
  • Santa Clara County Public Health Agency

The records will be retained for 60 days and will not be stored in personnel files or other files segregated by an identifying particular such as name or employee identification number.

Submission of this information is mandatory and failure to provide the requested information when one is working at a UCSC property (owned and leased properties) during the ongoing pandemic may result in disciplinary action, up to and including dismissal from employment. Additionally, employees who do not participate in the mandatory symptom check will be sent home without pay. Employees may elect to use accruals to cover the absence and/or work remotely; however, approval of such leave or remote working arrangement will be at discretion of the employee’s department. This symptom check process is not applicable to contractors, visitors, volunteers or non-affiliates.
